Foliage/Grass/Tents/Ropes disappearing in-game | POST HERE
Having the same issue, started in Lunden (55 hours in). Fast travelled, reloaded saves, etc. Nothing has fixed it.
This issue really needs to be addressed ASAP because the game is near unplayable with it. -

I could be wrong, but exactly like a couple of the other users in this thread, this issue started during the Bleeding the Leech quest.
