Order of Ancients Medallion - too many in inventory

For some reason, since the patch, an ‘order of ancients’ medallion has appeared in my inventory in the prologue. Going back over my save games, it appears right from the start, i.e. after the bit where you are a kid.
If I start an entirely new game, however, it is not there, so it is clearly a bug.
Trying to read ahead without spoiling myself, this seems as if it has potential to trigger an important reveal before I have properly collected all medallions, right? I saw you need a certain number of these medallions to trigger a particular reveal. Is there any workaround I could use to not break my game?
It will really suck if I need to restart my game entirely to get rid of this bug.
