Can't even start game, error message pops up all the time game crashes on loading screen.

Game crashes repeatedly, I deleted some old data files, reinstalled the game and reformated the ps4 worked for a little while then went back to the same old thing. I tried doing it again and to no avail. I get a clanking noise every time I load the disk in the ps4, It will either give me an error saying "cannot continue the application the disk cannot be read, Make sure the disk is not damaged or dirty" (which it is not)or code number. I took it back to the game shop, had the guy behind the counter explained that is what happened to his version on the older version of the PS4, fat edition. He put it on the slim version in the store and it worked. He told me it was a problem with UBISOFT's game and he had to keep messing with it repeatedly to get it to work.

I believe the error is CE-30005-8. And yes, it is a fat model destiny bundle PS4
